North Hudson Community Action Corporation earns highest accreditation from Joint Commission, nation’s most respected health care accreditation organization 

North Hudson Community Action Corporation has earned the highest rating available to health centers from the Joint Commission, which awarded it the designation “Primary Care Medical Home” retroactive to the beginning of 2013.

In July, surveyors from the Joint Commission, the nation’s standard-setting organization that rates health centers, hospitals and other health facilities, visited the North Hudson Community Action Corporation health center in West New York. Over two days they talked with patients and staff, reviewed records, and examined equipment and assessed the overall sanitation and safety of the facility.

This week the Joint Commission announced North Hudson Community Action Corporation had received full accreditation for all services provided. Only three Federally Qualified Health Center corporations in New Jersey have earned this accreditation.

Launched in July 2011, the Primary Care Medical Home designation focuses on coordination of care – meaning patients and caregivers must work closely on meeting health goals set by patients. They must develop a trusting relationship based on the patient’s needs, culture, values and preferences. The treating team may consist of physicians, physician assistants, nurses, pharmacists, nutritionists, mental health workers, social workers and others.

Care must be coordinated across the broader health spectrum, meaning primary caregivers must remain in touch with patients even when they are admitted to hospitals or are receiving treatment at home or in a long-term-care setting. Evidence-based medicine and clinical decision support tools, performance measurements, and patient satisfaction surveys are all used by Joint Commission surveyors when they evaluate a health center’s application for certification.

Dr. Jorge Verea and Dr. Carmen Mallamaci, Medical Directors of North Hudson Community Action Corporation , said they were pleased to receive the new designation and expressed thanks to their caregiving teams for joining them in the effort to attain the highest standard.
