NHCAC provides services to one of the most heavily populated areas per square mile in New Jersey. These services include psychiatric and physiological evaluations, individual and group therapies, prenatal and domestic violence counseling, addictive services, parenting skills training and academic guidance. THIS JOINT NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY. (A) INTRODUCTION This Joint Notice is being provided to you on behalf of NORTH HUDSON COMMUNITY ACTION CORPORATION (the “NHCAC”) and the employees and practitioners that work at the NHCAC with respect to services provided at NHCAC (collectively referred to herein as “We” or “Our”). We understand that your medical information is private and confidential. Further, we are required by law to maintain the privacy of “protected health information.” “Protected health information” or “PHI” includes any individually identifiable information that we obtain from you or others that relates to your past, present or future physical or mental health, the health care you have received, or payment for your health care. We will share protected health information with one another, as necessary, to carry out treatment, payment or health care operations relating to the services to be rendered at the NHCAC facilities. As required by law, this notice provides you with information about your rights and our legal duties and privacy practices with respect to the privacy of PHI. This notice also discusses the uses and disclosures we will make of your PHI. We must comply with the provisions of this notice as currently in effect, although we reserve the right to change the terms of this notice from time to time and to make the revised notice effective for all PHI we maintain. (B) PERMITTED USES AND DISCLOSURES We can use or disclose your PHI for purposes of treatment, payment and health care operations. For each of these categories of uses and disclosures, we have provided a description and an example below. However, not every particular use or disclosure in every category will be listed. Treatment means the provision, coordination or management of your health care, including consultations between health care providers relating to your care and referrals for health care from one health care provider to another. For example, a doctor treating you for a broken leg may need to know if you have diabetes because diabetes may slow the healing process. In addition, the doctor may need to contact a physical therapist to create the exercise regimen appropriate for your treatment. Payment means the activities we undertake to obtain reimbursement for the health care provided to you, including billing, collections, claims management, determinations of eligibility and coverage and other utilization review activities. For example, we may need to provide PHI to your Third Party Payor to determine whether the proposed course of treatment will be covered or if necessary to obtain payment. Federal or state law may require us to obtain a written release from you prior to disclosing certain specially protected PHI for payment purposes, and we will ask you to sign a release when necessary under applicable law. Supportive Services for Veterans Families
Committed to Ending Veteran Homelessness (View our community flyer HERE) TO LEARN MORE CONTACT 201-210-0101 SUPPORTIVE SERVICES FOR VETERAN FAMILIES (SSVF) PROGRAM The SSVF Program focuses on securing or maintaining existing housing for Veteran households who are currently homeless or are at risk of losing housing and becoming homeless “but for” SSVF assistance. Client-centered case management and other services (employment, housing counseling, financial education, health and dental care, mental health, substance abuse counseling, etc.) are provided to ensure Veterans receive the ongoing attention and care they need. Short-term financial assistance (legal, child care, transportation, security payments, rental assistance, utility payments and moving costs) may be available to eligible Veteran Families to assist in gaining housing stability and remaining stably housed after this temporary assistance ends. SSVF Eligibility Requirements: Individual/Head of household or his/her spouse must be a United States Veteran who served in active military, naval, or air service and discharged or released under conditions other than dishonorable. Individual/Family must fall below 50% of Area Median Income. HOMES FOR HEROES (HFH) PROGRAM The HFH Program provides housing and supportive services on a long-term basis for homeless veterans with disabilities (mental illness, problems with alcohol and/or drugs, physical, and acquired immune deficiency syndrome (AIDS) or related diseases) and their families. Families are housed in an 18-unit permanent supportive housing development in Union City, NJ. HFH Eligibility Requirements: Applicants must have documented proof of a long-term disability resulting mainly from serious mental illness, alcohol and/or drug abuse, or an HIV positive medical condition. Must be motivated for case management services. Must have an honorable discharge and be eligible for VA services.
