NHCAC
Provide primary and preventative care for patients and ensure Standard of Care for the patient within the Internal Medicine Group. Prescribes medications and therapies. Counsels and educates patients. Analyze and interpret data, formulate diagnosis and problem lists, and establish plans to manage common and complex chronic conditions, and health problems. Treat problems within their scope of competence and exercise judgment on problems requiring consultation, referral, or evaluation. Contributes to quality performance measures, regulatory compliance, training programs, and research initiatives. Precepts and collaborates with other staff members/disciplines to improve the patient experience. Participates in team meetings and activities of PCMH (Primary Care Medical Home) directly or indirectly. Enters into Joint Collaboration Agreements with one or more NHCAC Advance Practice Nurses.
Job Requirements: Active Medical License in the State of New Jersey; Board Certification in Adult/Internal Medicine. Current BCLS, Current DEA Certificate.
