The Referral Tracking Navigator (RTN) assists with coordinating patient healthcare needs, connecting patients with NHCAC resources and physicians. The RTN will monitor and track all internal and external referrals. This includes but is not limited to specialty care, ancillary services, health education, and behavioral health services.

High School Graduate or equivalent. CMA/MA Certification preferred. Minimum of two years’ experience in a health care setting. Knowledge of simple medical terminology. Bi-lingual English/Spanish preferred.

Dental Hygienist

Under the general direction of the dental provider, supports dental care delivery by cleaning and removing calculus and plaque (hard and soft deposits) from all surfaces of the teeth and below margins of gums, using dental instruments. Feels lymph nodes under patient’s chin to detect swelling or tenderness that could indicate presence of oral cancer. Feels and visually examines gums for sores and signs of disease. Applies preventive materials to the teeth (e.g., sealants and fluorides), teaches patients appropriate oral hygiene strategies to maintain oral health; (e.g., tooth brushing, flossing and nutritional counseling)

Current State of New Jersey Registered Dental Hygienist. Two year college degree in Dental Hygiene, Radiology license eligible

RN Clinical Instructor – Information Technology

Reporting to the Executive Vice President, the RN Clinical Instructor will serve as a clinical resource for clinicians to ensure the proper and effective use of the electronic medical records systems ensuring the improvement of the delivery of quality patient care. Is responsible for the training of new Physicians, NP’s, Nursing, Ancillary and other identified support staff; will act as the clinical liaison in assisting users with template navigation, tracking, troubleshooting and patient monitoring issues.

Will have added reporting responsibilities to the Director of Nursing for matters pertaining to direct patient care, patient care workflows and work plans for each discipline.  

EDUCATION AND EXPERIENCE REQUIRED:

NJ State Licensed Registered Nurse, BSN preferred. Experience working with EMR systems, templates and patient work flows. Experience in teaching/training the adult learner with differing levels of base knowledge from new hire to senior versed staff.

Annual Base Salary: $90,000, Full-time 40 hrs. per week / Exempt, Monday – Friday 8:30am-5pm / Rotation of Weekends/ Late Nights as assigned. Required to travel to all NHCAC sites and assigned locations within Bergen, Hudson and Passaic counties. Must have own means of transportation.

Infection Control Nurse Coordinator

The Infection Control Nurse Coordinator is responsible for the effective direction, management and operation of infection prevention and control/epidemiology, including education of NHCAC personnel. Collaborates with the CDC, and the county and state health departments. Responsibilities encompass compliance with Joint Commission (JC) standards, CMS, OSHA, and state regulations, outbreak investigation, and overall direction of the infection prevention and control program.

Must be willing to travel to all NHCAC sites and locations within Bergen, Hudson and Passaic counties.

EDUCATION AND EXPERIENCE REQUIRED:

Requires a current NJ RN license with BSN preferred, OR Masters in Public Health (MPH), OR a bachelor’s degree in microbiology or epidemiology.

Must have a minimum of five years clinical experience in a healthcare/center setting or disease surveillance experience in a public health setting.

Association for Practitioners in Infection Control (APIC) training course or equivalent desired. Certification in Infection Control (CIC) is preferred.

Computer skills, research and basic statistical is preferred.

Sterile Processing Technician

The Sterile Processing Technician is responsible for performing decontamination, cleaning, assembling, packaging, logging, sterilization, storage and distribution of reusable surgical instrumentation and equipment.  Able to travel to all health center locations within Bergen, Hudson and Passaic counties. Own means of transportation.

Required Certifications, Education and Experience:

·         Must have a valid current Certified Registered Central Service Tech (CRCST) certification.

·         High School diploma or general education degree (GED) required.

·         1-2 years active experience in the Sterile Processing field.

Outreach Assistant

The Outreach Assistant participates in the development, implementation and coordination on a variety of outreach and enrollment programs designed to identify under-served population segments and provide assistance for accessing services. Represents NHCAC as a member of the Outreach team. Aids in the development of new collaborative efforts and partnerships goals of NHCAC and its members as directed by EVP.

EDUCATION AND EXPERIENCE REQUIRED: High School diploma or general education degree (GED); community healthcare experience a plus. Certification as counselor for open enrollment in the market place or must secure certification within 90-days of employment. Bi-lingual English/Spanish preferred

The Non-Public School Nurse (RN) is a pivotal role providing expertise and oversight for the provision of school health services.   Using clinical knowledge and judgment, the school nurse provides health care to students and staff. The school nurse serves as a liaison between school personnel, family and community.

Valid NJ State Licensed Registered Nurse, CPR & AED current certification. Bi-Lingual English/Spanish a plus. Own means of transportation.  Per diem, 2-3 days per week.

Behavioral Health Community Health Worker

The Community Health Worker (CHW) will be responsible for helping patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW supports the health centers through an integrated approach to care management and community outreach. As a priority, activity will promote, maintain, and improve the health of patients and their family. Provide social support, psycho-education, advocate for individuals and community health needs, such as community outreach, and health screenings.

The  Registered Nurse (RN) is responsible for diagnosing and treating human responses to actual or potential physical and emotional health problems, through such services as casefinding, health teaching, health counseling, and provision of care supportive to or restorative of life and wellbeing, and executing medical regimens as prescribed by a licensed or otherwise legally authorized physician or dentist. Diagnosing in the context of nursing practice means that identification of and discrimination between physical and psychosocial signs and symptoms essential to effective execution and management of the nursing regimen. Such diagnostic privilege is distinct from a medical diagnosis. Treating means selection and performance of those therapeutic measures essential to the effective management and execution of the nursing regimen. Human responses means those signs, symptoms, and processes which denote the individual’s health need or reaction to an actual or potential health problem. (NJBON Statutes: Title 45; Chapter 11). Under the direction of the Nursing Coordinator and/or Provider, the RN is responsible for the delegation of staff and delivery of patient care.

Under the direction of a Registered Nurse and/or Provider, the Licensed Practical Nurse (LPN) is responsible for performing tasks and responsibilities within the framework of case finding reinforcing the patient and family teaching program through health teaching, health counseling and provision of supportive and restorative care (NJBON Statutes: Title 45; Chapter 11). This includes, but is not limited to the following elements of patient care: planning, and implementation of the plan of care and evaluation in an ambulatory office setting.
