This role includes processing medical records requests, maintaining organized patient records, and ensuring compliance with HIPAA and NJ PRA guidelines. The position also supports obstetric PRA documentation, data entry, follow-up tracking, and collaboration with clinical staff to ensure timely and accurate completion of PRA requirements.

High School Diploma or GED required; 1+year(s) related experience and/or education in the Health Information Management field. Experience in an Ambulatory Care setting, a plus.

HUD Certified Housing Counselor

Under the direction of the Vice President of CAP Services, the HUD Certified Housing Counselor provides housing counseling, education, and advocacy to clients throughout Hudson County. The role supports individuals and families in achieving housing stability, financial readiness, and sustainable homeownership.

Key Responsibilities: Provide one-on-one and group counseling in homeownership, rental, and homelessness prevention. Educate clients on budgeting, credit, fair housing laws, and tenant/landlord responsibilities. Assist clients in developing individualized action plans and connect them with community resources. Maintain accurate client records and prepare required program reports. Stay current on housing laws, regulations, and best practices.

Education and Experience Required: HUD Certified Housing Counselor Certificate issued by the U.S. Department of Housing and Urban Development (must include issue date and Housing Counselor ID number). Bachelor’s degree in Business Administration, Social Sciences, or a related field. Additional certifications in case management, credit counseling, or related areas preferred. Strong communication, organizational, and case management skills. Bilingual (English/Spanish).

Housing Case Manager

The Housing Case Manager supports individuals and families experiencing homelessness in securing and maintaining permanent, affordable housing. Through comprehensive case management, assesses participant needs, identifies and addresses barriers to housing, and develops individualized housing stabilization plans that promote long-term self-sufficiency. This role works collaboratively with internal staff and community partners to connect participants to essential resources, monitor progress, and ensure program compliance. Maintains accurate documentation and reporting in accordance with funding and regulatory requirements.

Qualifications: Bachelor’s degree in Social Work, Counseling, or a related field, or equivalent experience (minimum 5 years in social services) Experience working with vulnerable populations, preferably individuals experiencing homelessness Strong knowledge of community resources and housing programs

Under the supervision of the Chief Nutritionist, the Nutritionist provides nutrition assessments, counseling, and food package prescriptions for women, infants, and children participating in the WIC Program, in accordance with organizational and New Jersey Department of Health policies and procedures. The Nutritionist serves as a Competent Professional Authority (CPA), determines nutritional risk, provides nutrition and breastfeeding counseling, assigns appropriate food packages, and makes referrals to health and social services as needed.
